All-on-4 Dental Implants: A Fixed Full Arch on Four Implants

All-on-4 is a treatment concept in which a complete fixed dental arch is supported by four strategically positioned implants in many, but not all, cases. It may be considered when an upper or lower jaw has no teeth, or when the remaining teeth cannot be maintained with a predictable long-term outlook. The objective is broader than filling spaces: planning must restore a stable bite, speech, facial support and a smile that remains cleanable. Suitability can only be confirmed after a clinical examination and appropriate imaging.

What does All-on-4 actually mean?

The rear implants may be tilted to make efficient use of available bone and reduce the length of the unsupported bridge. That does not mean four implants are automatically ideal for every jaw. Who may be a suitable candidate?

Planning usually includes photographs, a panoramic X-ray and often CBCT imaging, together with impressions or scans and a bite assessment. Teeth that cannot be retained are removed and the implants are placed in their planned positions. If primary implant stability and other clinical conditions are satisfactory, a fixed provisional bridge may be fitted quickly; immediate loading is never an unconditional promise. After osseointegration, new records and try-ins are used to refine tooth shape, colour, speech and bite before the definitive bridge is produced.

Benefits and limitations to consider

A fixed full arch does not need to be removed for meals and can feel far more secure than a conventional denture. The palate can remain uncovered, and facial support can be designed into the restoration. The procedure nevertheless involves surgery, a healing period and strict hygiene. The provisional bridge must be protected from excessive loads. Possible complications include inflammation around an implant, loss of integration, screw loosening, chipping or wear of prosthetic teeth. Prompt review and a planned maintenance programme help manage these risks.

What determines the cost of All-on-4?

Cost depends on whether one or both jaws are treated, the diagnostic work, extractions, any grafting, sedation where appropriate, the provisional bridge, the definitive material and follow-up care. A responsible fixed price cannot be given before examining the mouth and scans. Cleaning and long-term maintenance

All-on-4 teeth are fixed, but they are not maintenance-free. Plaque must be removed from the space beneath the bridge every day with floss threaders, interdental brushes, an oral irrigator if recommended, or other tools demonstrated by the dental team. Reviews assess the soft tissues, bone levels, bite, screws and bridge material. Professional cleaning, smoking cessation and management of grinding can all improve the outlook. Prosthetic components may need servicing or replacement over time even when the implants themselves remain healthy.

Frequently asked questions

Can I really receive fixed teeth in one day?

Some patients can receive a fixed provisional bridge on or soon after surgery. This requires adequate implant stability and a favourable clinical situation. The definitive bridge is normally made after healing and detailed reassessment.

Is All-on-4 surgery painful?

Local anaesthesia is used, so the procedure itself should not be painful. Swelling, tightness and tenderness are expected afterwards and vary between individuals. Follow the prescribed medicines and contact the clinic if symptoms worsen unexpectedly.

Will I need a bone graft?

Not always. Angled implants may avoid grafting in selected cases, but they cannot eliminate the need in every anatomy. Three-dimensional imaging is required to assess available bone and nearby structures.

What can I eat after treatment?

A soft diet and gentle, balanced chewing are normally required during early healing. A provisional bridge cannot be treated like fully integrated final teeth. Your clinician will provide instructions tailored to implant stability and healing.

How long does an All-on-4 bridge last?

Implants can function for many years with good health and maintenance, but no lifetime guarantee is possible. Teeth, acrylic or ceramic surfaces, screws and other prosthetic parts may wear and require repair or renewal.
